Coordinated Care

You Benefit From Close Cooperation Between Primary Care Providers and Specialists

Heart care is coordinated between members of the team and your regular doctor.
Heart care is coordinated between members of the team and your regular doctor.

It all starts with your primary care provider, who will refer you to an appropriate heart specialist if needed. These specialists have dedicated their practice specifically to diagnosing and treating heart disease. The team also includes rehabilitation specialists, technicians, nurses, emergency department doctors and other staff involved in a comprehensive web of care.

Care is provided by the team at:

  • Marshfield Clinic centers
  • Ministry Medical Group clinical offices
  • Ministry Health Care hospitals
  • Urgent care, walk-in clinics and emergency departments

Virtually all heart patients can get the care they need entirely within the Marshfield Clinic and Ministry Health Care systems, making electronic patient records accessible, keeping protocols of care consistent, minimizing delays, simplifying paperwork and keeping patients closer to their friends and family.

We provide heart care at over a dozen locations throughout central and northern Wisconsin, click here to learn more.
