Ministry Health Care, part of Ascension, is pleased to welcome Colleen Fraley, FNP-C, to Ministry Medical Group’s family medicine department. Fraley sees patients at 930 East Wall Street in Eagle River.
A board-certified family nurse practitioner, Fraley provides primary healthcare services to patients who are 9 months of age and older. She provides care throughout all stages of life and has special interest in preventive care, the treatment of acute injuries and the management of acute and chronic illnesses. Her goal is to establish long-term relationships with her patients and their families. She helps her patients understand their disease processes and collaborates with them to make healthcare decisions that align with their personal values as they achieve optimal health and wellness.
Fraley received her associate nursing degree from the Health Sciences Center in Canada. She earned a Bachelor of Science in nursing from Chamberlain College of Nursing in Downers Grove, Illinois. She completed her Master of Science degree in nursing at Concordia University in Mequon, Wisconsin.
“I believe in utilizing education to empower my patients to make informed decisions about their healthcare,” said Fraley. When she’s not in the office, Fraley enjoys camping, hiking and kayaking.
