December 5, 2011 is Deadline for Funding Applications for MGSHC Foundation

The Ministry Good Samaritan Health Center Foundation is pleased to announce that “Applications for Funds” are being accepted for its 2012 community allocations.

In order for a project to be funded, it should demonstrate alignment with the mission and values of Ministry Good Samaritan Health Center and its sponsor Ministry Health Care which is to further the healing ministry of Jesus by continually improving the health and well-being of all people, especially the poor, in the communities we serve. This mission is fulfilled on the founding values of service, vision, justice and presence.

Funds will be given to 501c3 organizations or programs that support healthy initiatives within the Merrill community.

In addition to mission and values alignment, the project should address one or more of the following objectives:

• Supports a priority health need in the service area as identified through research and/or outcomes measurement.
• Enhances the quality of care and services provided for local programs or services that address identified health needs in the community it serves, especially for the poor and vulnerable.
• Promotes public awareness of health information to prevent illness, accidents, injuries and disease in the service area of Ministry Good Samaritan Health Center.
• Provides education to help everyone understand how healthy we are and what are the most important changes we can make to improve our own health as well as the health of our families and communities

All applicants must complete a “Ministry Good Samaritan Foundation Application for Funds” and attach appropriate documents. The Foundation’s Planning/Development Committee and Foundation Director will review all requests with a recommendation then forwarded to the Foundation Board for its approval.
