Welcome to Ministry Health Care’s Student Placement Website!

Ministry Health Care offers a closer look at health care careers through a student placement which is either a requirement from an educational institution or certification board.

This information is designed to assist instructors and students who seek clinical placements at Ministry Health Care. If you have any questions, please contact Student Affiliations at studentaffiliations@ministryhealth.org.

Instructor Placement Requirements:

1. Master Student Affiliations Agreement - Ministry Health Care requires a signed master student affiliations agreement prior to requesting a student placement. This signed master student affiliations agreement is between Ministry Health Care and the school the student attends. Ministry Health Care utilizes a master student affiliations agreement which is unique to Ministry Health Care.

  • Prior to entering a student placement request, please click here to validate your school is listed in the database which will ensure Ministry Health Care has a current master student affiliations agreement signed. If your school is not listed, please follow the instructions in the database to initiate a master student affiliations agreement. This can be a time intensive process - a typical timeframe may be 30 to 60 days. Please submit a request immediately for any upcoming placement requests that need to be completed.
  • Ministry Health Care will not begin to process any student placement requests without a signed master student affiliations agreement.

2. Instructor Requirements – every onsite instructor leading student(s) at a Ministry Health Care location are required to complete the same requirements as a student.

  • Please sign the Student/Instructor Confidentiality Form. This form should be kept on file with the school.
  • Each student/instructor is required to have a completed Background Check prior to beginning a student placement at Ministry Health Care. The Background Information Disclosure form can be found here. It is the school’s responsibility to complete and log appropriately in the student record within the student placement request. Please note additional information on background checks in the Instructor/Requester Student Placement Request Process below.

3. Instructor/Requester Student Placement Request Process:

  • To submit a student placement request, the instructor/requester will need a username/password to access the Ministry Health Care – Student Placement Request database. If you do not have a username/password, please send an email to Studentaffiliations@ministryhealth.org with the information below. You will receive an email granting you access to the Ministry Health Care – Student Placement Request database.
    • Information required in email: Full Name, Name of School, Program Name, Email Address, Phone Number
  • Upon receipt of the email from the Ministry Health Care – Student Placement Request database, the instructor will login to the Ministry Health Care – Student Placement Request database and complete a Student placement request using the directions listed in the database. If needed, please click here for screen shots which will walk you through the student placement request process.
    • The Student Affiliations Coordinator will send the request to the appropriate Ministry Health Care leader for approval/denial of the student placement request.
    • The instructor will be notified when the student placement request has been approved or denied.
  • Within each student placement request, the instructor will need to list the student(s) information including completion of the Ministry Health Care requirements (health work, orientation, confidentiality, authorization, background check). Ministry Health Care does not require the requirement paperwork to be submitted unless there is a positive finding on the background check.
    • If there is a positive finding on the background check, the instructor is required to submit the completed Background Information Disclosure (BID) form with the background check results to studentaffiliations@ministryhealth.org. After further investigation by Ministry Health Care, the Student Affiliations Coordinator will notify the instructor if the student is approved or denied participation of the student placement with Ministry Health Care.     • The student will not be “cleared” for the placement until all requirements have been “checked” off by the instructor of the participating school and the Student Affiliations Coordinator has reviewed the placement request and student information for completion. Once this occurs, the instructor will receive an email indicating the student is “cleared” for the placement. Students are not allowed begin a placement until this “clear” email has been received.

Student Placement Requirements:

Students will not be required to complete any information in the Ministry Health Care – Student Placement Request database. But, students should know, understand and complete the following requirements for student placements with Ministry Health Care and have documentation of completion on file with their school.

  • Please also sign the Student/Instructor Confidentiality Form. This form should be kept on file with the school.
  • Each student is required to have a completed Background Check prior to beginning a student placement at Ministry Health Care. The Background Information Disclosure form can be found here.   • Per the student placement, a valid and non-expired Basic Life Support certification issued by the American Heart Association may be required.
