Allied Health Professions and Nursing Students

This information is designed to assist students and instructors who are seeking a clinical rotation or internship placement at Ministry Health Care/Affinity Health Care. Instructor Section

1. An affiliation contract must be in place prior to the start of placement request. If you are unsure whether or not an affiliation agreement exists between your college and Ministry Health Care, please contact Laura Bart, Student Affiliation Coordinator at Ministry/Affinity Health Care.
2. If you have confirmed that an affiliation agreement is in place, please complete the Placement Request Form to request placement for your student or student group. Your request is automatically submitted to the Student Affiliation Coordinator for the approval process. You will be contacted when your request has been approved.
3. All students have a list of requirements that need to be met prior to the start date of their placement. To assist you with the current requirements for students participating in clinical placements please access the Requirements Form.
4. A verification form must be completed for each student and sent to the Student Affiliation Coordinator prior to the start date of the placement. Please click here for the verification form.
5. An on-site unit orientation must be completed for each student and given to the unit manager to keep on file in the department. Please click here for the Student Unit Orientation.

Student Section

1. Complete the on–line Student Application. You will need your placement information and instructor information. When you save your form it will automatically be submitted.
2. Complete the Orientation.
3. Complete the Student Requirements Information Sheet.

When all the requirements have been met, a student name tag will be sent to either your instructor or preceptor. This must be worn behind your college photo ID while you are participating in clinicals or rotations at Ministry/Affinity Health Care.

NURSING STUDENTS:

If you are a nursing student who will participate in a clinical at Ministry Saint Clare’s Hospital, Ministry Saint Joseph’s Hospital or Ministry Our Lady of Victory Hospital. Please review the appropriate computer charting training below.
