Ministry Health Care (MHC) is pleased to partner with you for clinical education experiences.

School representatives: This page is designed to help you through the placement process at MHC. Please review and complete Steps 1 – 4 in the placement process outlined below. Email Student Affiliations with any questions: studentaffiliations@ministryhealth.org.

Students: Go to Step 4: Student Placement Requirements. Complete and submit the required documents to your school representative and they will record in the MHC Student Placement Request Database. Note: Nursing Students at some MHC locations have also have computer based trainings (CBTs) to complete prior to placement. Please scroll to the section, Nursing Students to view the site specific nursing CBTs.



Clinical Placement Requirements:

Step 1 - Master Student Affiliations Agreement

MHC requires a signed master student affiliations agreement with the school the student attends prior to requesting student placements.

Please click here to confirm that your school is listed in the MHC agreement database. If your school is not listed, please follow the instructions in the database to begin the agreement process. This can be a time intensive process. Please submit a request immediately for any upcoming placements.

Step 2 - School Representative Placement Request

To submit a placement request, the school representative will need a username and password to access the MHC Student Placement Request database. If you do not have a username/password, please send an email to studentaffiliations@ministryhealth.org to request. Please include the following in your email message:

Name of School
Name of Program
School Contact Name
School Contact Email
School Contact Phone

 

Upon receipt of the email with username and password information, the school representative can log into the Ministry Health Care – Student Placement Request database and complete a placement request. Please click here for screen shots which will walk you through the placement request process.

Entering Student and Instructor information into the database: If your request has been approved, the school representative lists the instructor (if it is an instructor led clinical) and student information. Compliance with the MHC requirements (health, safety, orientation, confidentiality, authorization, background check) is documented within the database.

Background Check Process: If there is a positive finding on the student or instructor background check, the school representative is required to submit the completed Background Information Disclosure (BID) form with the background check results to studentaffiliations@ministryhealth.org. After further review by MHC, Student Affiliations will notify the school if the student is approved or denied participation.

  • The instructor/student will not be “cleared” for the placement until all requirements have been “checked” off by the school representative and Student Affiliations has reviewed that requirements have been met. When this step is complete, the school representative will receive an email indicating the instructor/student(s) are “cleared” for the placement. Placements may not start until participants are cleared.

Step 3 - Instructor Requirements

Clinical instructors leading student(s) at a Ministry Health Care location are required to complete the requirements found in the steps below. Document completion and keep forms on file at the school.

  1. MHC Health Requirements. Keep documentation of health requirements on file at the school.
    • Hepatitis B Declination Form: please click to read this declination form and the Hepatitis B Vaccination Information Statement.
    • TB Screening Form: there are some instances when this form is completed and kept on file. Please learn more from the MHC Health Requirements.
  2. Student/Instructor Confidentiality Form: Sign this form and keep on file at the school.
  3. Complete Orientation Courses:
  4. Read and sign the Student/Instructor Authorization Form: This form is to be kept on file at the school.
    • Each unit/department may have different requirements for orientation. Each student/instructor is required to complete a Student/Instructor Unit Orientation Checklist at the start of each placement request. This form is to be kept on file with the school.
  5. Background Check: Each student/instructor is required to have a completed Background Check prior to beginning a student placement at MHC. The Background Information Disclosure form can be found here. It is the school’s responsibility to complete and log appropriately in the student record within the student placement request.
  6. Per the student placement, a valid and current Basic Life Support certification issued by the American Heart Association may be required.
  7. Current RN License for Nursing Instructors

Step 4 - Student Requirements

Students complete the following requirements for placements with Ministry Health Care. Follow the steps below and provide completed documents to your school representative.

  1. MHC Health Requirements.

  2. Sign the Student/Instructor Confidentiality Form. This form is to be kept on file with the school.
