Whatever sports your children play this summer or fall, they may need a sports physical to meet the participation requirements of their school or league. Below you will find specific dates, times and locations where Ministry Medical Group is offering Sport and Camp Physicals.

Registration forms need to be picked up at your local school. Appropriate form sections must be completed prior to arrival. A parent/guardian will need to accompany their child to their scheduled time. The WIAA physical is not intended to replace regular check-ups with your child’s physician. Students that have had a preventative exam within the past year their pediatrician/family medicine clinician can complete the WIAA form without an any additional exam.
 

Sports physical clinics are meant as a service to comply with WIAA guidelines and are not meant to replace a comprehensive annual preventative health exam.
