Regular monitoring of clinical and operational processes helps ensure compliance with federal, state and accrediting bodies. Ministry Home Care’s agencies are certified by the Centers for Medicare & Medicaid Services (CMS), and licensed by the State of Wisconsin.
Performance Improvement activities may be initiated any time a clinical or operational process is found to be ineffective. A team is identified to evaluate the process and develop an action plan to promote more effective outcomes.
Satisfaction surveys target patients, clients, physicians and referral sources.
Voluntary benchmarking activities allow our individual agencies to compare themselves to other home care agencies. Ministry Home Care partners with SHP to benchmark on a real-time basis. To see our latest results, click here.
Home Health Compare is a CMS–sponsored Web site that compares agencies on specific patient outcomes with a national reference group and with other agencies in their state.
Ministry Home Care partners with a nation-wide quality initiative to decrease the re-hospitalization of home care patients and to improve patient medication management.
