Home-based monitoring helps your professional caregivers get daily information about your health. Daily monitoring can include using small wireless devices set up in your home to perform one or more of the following activities each day:
- Answer a few questions about how you feel.
- Measure your
- Blood pressure
- Heart rate
- Blood oxygen level
It’s easy to do, and can contribute significantly to improved health.
With this program, our home health care team receives daily reports of your vital signs more frequent than your doctor’s office visits. This daily information is a useful supplement to the care you get from your doctor – it is not a replacement for that important care.

Ministry Home Care will deliver everything you need to your home and teach you how to use it. This program is provided for you at no extra cost.
Our telemonitoring system requires just a few minutes each day.
The system we use was designed for ease of use. It is simple for patients to take these measurements without assistance. The readings are sent automatically through your telephone line (toll-free) so our agency nurse can review them each day. There are no interruptions to your telephone service.
