Ministry Home Care's Acclaimed Facility Training Program

With the latest Conditions of Participation outlined by Medicare, it was important to develop a training program that would meet the needs of our contracted facilities as well as the demanding schedule of the hospice team. This program incorporates two professionally-produced video productions and a complementary Learner Guide to reinforce the key points covered in the DVDs.
We carefully incorporated the points outlined in the Medicare Conditions of Participation into the program, and present them in a context that is easily understood. Each DVD is fewer than 20 minutes in length, and incorporates the powerful music of Grammy-nominated songwriter and singer Karen Taylor-Good. The Learner Guide comes on a CD for the facility to print as they need copies for new employees.
The videos may be purchased in a basic format or with your organization's logo. If customized with your logo, you have the option to include the logo on the cover art only, or within the body of the video presentation itself. Like all of our videos, once you've purchased the initial copy, you can purchase additional copies of the same level for a very low reproduction cost. Even if it's many months from now - you pay only the reproduction cost moving forward.

Part 1: Hospice: Navigating the Final Journey
This 18 minute program gives a strong overview of hospice, providing information that is valuable to anyone in the community. As an introduction to the model of care, this video lays the groundwork for educating facility staff. A very valuable tool that also works well in community outreach presentations, volunteer training and support staff inservices.
Part 2: Hospice, Families and Care Facilities: A Life-Giving Partnership
A nice follow-up to the first DVD, this program provides information specific to individuals working in a facility. How the hospice team and facility professionals collaborate and interact, division of roles and responsibilities, and processes to follow are all covered in this 15 minute production. Filmed in actual care facilities and utilizing professionals from both entities, the program gives a clear picture of how the teams can work together to provide the best care possible for the residents. Valuable for use in training hospice staff and volunteers as well.
Learner Guide (printable files on CD)
This 10-page booklet can be printed by the facility as needed for each of their employees. Highlighting many of the key points of the program, the Guide reinforces the learning of the participants. Topics include:
