We have exciting news to share!

Ministry Medical Group will begin using an innovative automated health management program designed to help us deliver better coordinated, more personalized care to you and your family—inside and outside of the office. Our goal is to ensure that you receive the care you need, when you need it.

How the Program Works

We have initiated a state-of-the-art program that will regularly track your primary care needs and send you a pre-recorded phone message when you are due for necessary services.  You will receive reminders customized to your individual needs. Examples include reminders for existing appointments, annual physicals, well child visits, preventive services, cancer screening and disease management reminders for diabetes and cholesterol. Preventive services now will lead to better health and lower health care costs for you in the future. We believe you will benefit from the reliability, convenience and effectiveness of our new program.

Be assured that, because of patient privacy, we won’t provide detailed medical information in our messages. We’ll request that you call our office and then one of our staff members will explain your recommended care.
 
When it is being rolled out

This program is being rolled out into phases across all Ministry Medical Group outpatient clinics in our service area. This program is being offered for internal medicine, family medicine, pediatrics, and woman's health. The following roll-out plan is:

• Phase I - Plover, Rhinelander (Riverside Campus), Woodruff
• Phase II - Amherst, Crandon, Eagle River, Iola, Laona, Rhinelander (Northshore Campus), Stevens Point, Waupaca
• Phase III - Merrill, Rib Mountain, Tomahawk, Weston

We appreciate your patience during implementation of this new program. If you receive a call that is confusing to you or if you think we called you in error, please let us know as soon as possible so we can address your concern or question immediately. Your records may be in need of updating.  For example, we may find that we have a wrong birth date for you in our system or that you have received needed preventative services at another clinic for which we lack documentation.
