A community survey to gather public input on health behaviors and practices will be distributed throughout Lincoln County starting May 1. The survey is part of Healthy People Lincoln County’s (HPLC) Community Health Improvement Planning Process for 2012. It was developed by a subcommittee consisting of Ministry Good Samaritan Health Center, Ministry Sacred Heart Hospital, Lincoln County Health Department, Jester Studio, and Lincoln County-UW Extension.
“Gathering public input on their health is a very important part of developing a plan for the future”, said Anne Mathson, Foundation and Outreach Director, Ministry Good Samaritan Health Center. In order to develop a plan that will be owned and used by the community we are asking for public input throughout the planning process”, she added.
Surveys will be available at Ministry Good Samaritan Health Center, Ministry Sacred Heart Hospital, Lincoln County Health Department and various locations throughout the county. Click here to complete the survey online. It should take about 15 minutes to complete. Those who fill out the survey are eligible to put their name into a raffle drawing for a $20 Chamber Gift Card. The survey is confidential and anonymous.
“We are asking people about their health to form a baseline of information,” Paula Gebauer RN, Clinical Quality Improvement Specialist, Ministry Sacred Heart Hospital said. “We are hoping over the next several years, with enough focus on the health priorities, we will see an increase in healthier behaviors and practices.”
Healthy People Lincoln County Plan is updated every five years. The 2012-2017 plan priorities are oral health, mental health, and nutrition. We invite you to actively support Healthy People Lincoln County’s Community Health Improvement Plan. If you are interested in volunteering, we encourage you to help with an event or be a member of an area coalition. You can also partner with your family, worksite, or a community organization that you are part of to promote oral health, mental health, and good nutrition.
