Welcome to the Ministry Health Care Student Affiliations web site.

The information provided here is designed to guide students who are seeking a learning experience with Ministry Health Care. Students must attend an institution of higher education and be enrolled in a program requiring applied learning objectives to be able to participate in a clinical or educational placement.

An Affiliation Agreement (Contract) must be in place between the school and Ministry Health Care. Please contact the Student Affiliations Coordinator at 715-343-3373 if you have questions regarding an existing contract or need to initiate a new contract.

The institution of higher education must provide an instructor/coordinator to sponsor the student(s) and to act as a liaison to Ministry Health Care.

Placement Steps

  1. Requirements Form - Print, complete and return along with all necessary paperwork.
  2. Complete the on-line application form. Make sure you have your school, emergency contact, and placement information handy for you to refer to as this will be required on the application. PLEASE NOTE: A Ministry Student ID number will be assigned after completion, please keep this for any future placements you may need to add.
  3. Read the General Orientation Policy Booklet. This booklet is available in print form.
  4. After reading and agreeing to the terms of the General Orientation Policy Booklet, print, sign and return the General Orientation Policy Form.
  5. Submit your completed forms to our office at least 6 weeks prior to the desired start date of your placement.

Returning Students

Complete the Placement Form only. This form will ask you for your Ministry Student ID Number. Be sure you have it with you before you begin to fill in your placement request.

 

FREQUENTLY ASKED QUESTIONS:

I am part of a group of students going with my instructor to my clinical site. Do I still have to complete the on-line application?

Yes, any student completing a clinical learning experience within a Ministry Health Care facility must complete the on-line application.

How do I know if my individual placement request has been approved?

The Student Affiliations office will contact you by e-mail. Please be sure to have a current e-mail address submitted on the on-line application form.

I am not sure if I am immune to all of the communicable diseases listed. Can I still participate in a learning experience?

The Ministry Health Care Student Affiliation office in conjunction with Ministry Health Care's Employee Health department will evaluate any non-immune students and advise them on an individual basis. Unless contraindicated, you will have to receive a booster immunization and have a new titer drawn.If after receiving a booster immunization, your titers are still showing non-immune the Student Affiliation office will consult with you on an individual basis.

I will be attending numerous clinical rotations, do I have to keep submitting my paperwork?

No.Once you have submitted the appropriate paperwork, you will need only to complete a new placement information form and keep current with your TB skin tests.
